I can't shake the idea that "agitated doctor" might be a cryptic crossword-style clue. Does "agitated doctor" mean that we have to make an anagram out of the word "doctor", or out of a word for doctor?

Assuming no to the above, is a fictional doctor relevant? Doctor Who? Someone with a PhD but who isn't a medical doctor? Someone who's called a doctor but isn't one at all? Doc Holliday?
