I hosted a public panel discussion at work. One of the panelists, a co-worker who usually goes by Izzy, asked me to introduce her as "Isabella [Last Name]" as she thought using her full first name would make her look more professional. I'm so used to calling her Izzy that I had to focus really hard on saying "Isabella" instead and then completely blanked on her last name. I got a deer-in-headlights look until she came to my rescue, but luckily the audience just laughed it off and I was a lot less nervous after that.
