This comes from an account of silver mines in colonial South America, which were very unpleasant and unsafe places, and many of the workers were either slaves or impoverished indigenous people who couldn't make a living anywhere else.

The specific lack of foresight in this case was that the mines were often built simply as long tunnels straight down into the ground, even though people at the time knew that you should dig a mine at an angle with extra air shafts up to the surface--otherwise heavy gases sink to the bottom of the pit and push out all of the breathable oxygen.

In this incident, a worker fell into the hole, and there was so little oxygen at the bottom of the hole that his torch died almost immediately. People climbed down to help, but by the time they reached him he had asphyxiated.
